Social prescribing service working with people to identify their non-medical needs and supporting them in accessing the right services. The aim is to help people find out what's important to them, what changes they would like to make in their lives and help identify the best ways to achieve those. Examples include exercise, support groups, coffee mornings and financial and housing advice.
