Team of trained coaches who work with people to identify any non-medical needs and help them find the right support, including all aspects of physical health, mental health, staying in work, finding paid or non-paid work, housing, finances and feeling more connected. After an initial assessment they then arrange regular one-to-one sessions either face-to-face or over the phone. You can ask your GP to refer you, or contact them directly.
