What

12-week GP exercise referral scheme for Horsham District residents living with long-term health conditions, including those recovering from injury. The programme provides a personalised exercise prescription with one-to-one support from a trained Exercise Referral Co-Ordinator. Participants have access to the fitness suite, swimming pool and selected group exercise classes, with progress reviews every four weeks.

Who

Anyone who has been referred by their GP with a referral form submitted to the Horsham District Council Wellbeing team. Open to people with a long-term health condition, asthma, arthritis, high blood pressure, diabetes, obesity, depression, anxiety or osteoporosis. Patients must ask their GP or healthcare professional for a referral.
