Palliative Care Hub supporting adults living with life limiting conditions and their loved ones and carers. The Hub takes referrals and messages, answer queries and offers physical and mental wellbeing support including support and advice on symptom control and management. Support is given in hospital, in the community, and at its hospices, Sobell House and Katharine House. See website for more information.
