Hospice for adults living with life limiting conditions and their loved ones and carers. It offers palliative care services at the hospice, in the community or a person’s own home. Services include a specialist nurse-led team, hospice day care, emotional and psychological support, complementary and creative therapies, bereavement, carer and social support. Who

Adults aged 18 and above living with a life limiting condition, registered with a GP within Southend, Castle Point or Rochford, or residing in that area, referral can be made by self, carer, GP or healthcare professional, please see website for eligibility
