The service, open Monday to Friday from 8:30am to 4:30pm, offers pain and symptom management, end of life care, rapid discharge from hospital, advanced care planning, and support for complex psychological or emotional issues (patient/carer). People in the Wigan area, living with progressive life-limiting conditions. Any professional can refer, if patient consent has been obtained. Self referrals also accepted.
