Hospice for people living with life-limiting conditions and complex needs. It offers palliative and end of life care at the hospice, in people’s own homes or in the community. This includes pain/symptom management, a range of therapies, spiritual care and bereavement counselling. Support is also offered to chosen loved ones/ carers. Adults living with life-limiting conditions who are registered with a south Worcestershire GP, including Worcester, Droitwich, Pershore, Evesham, Malvern, Tewkesbury, Broadway, Martley and surrounding areas. Referral is made by health care professionals via an online referral form
