Referral programme for physically disabled adults who want to get more active and learn how to eat more healthily. It offers twelve weeks of support with food and exercise, in person or online (or both). You need to be referred by a health care professional to access. Physically disabled adults aged 18 and above living, working or studying in Tower Hamlets and Hackney, who are overweight or obese. Referral is via health care professional
