NHS community service to prevent unnecessary admission to hospital and facilitate discharge. Its multidisciplinary team identify, assess and co-ordinate admission avoidance/discharge in the emergency department and provides rapid follow up/response in the community to ensure the person's health and social care needs are met in the most appropriate place.
