Exercise referral scheme helping people living with long-term health conditions improve their fitness. This scheme is a partnership between Everyone Health and Southwark Council. It aims to improve your physical and mental wellbeing through gym, fitness classes and swimming. You must be referred by a health professional such as your GP to access.
