What Woking & Sam Beare Hospices is a charity that cares for patients and their carers and families. They specialise in delivering holistic care for patients with progressive advanced conditions that are life limiting. The hospice’s care is delivered in the community in patient homes, in care or nursing homes or through its in-patient ward in the hospice; as well as through the Bradbury Wellbeing Centre. Where Referrals can be made from six Surrey Boroughs [Spelthorn, West Elmbridge, Runnymede, Surrey Heath, Woking and North Guildford].
Who The service provides support to patients with advanced disease and to their families. Referrals need to come from a healthcare professionals, like GP on cancer nurse specialist to ensure it is appropriate and sent with all the relevant medical information.
