What

Hospice offering specialist palliative and end of life care for people living with life-limiting conditions. It supports people in their homes and at the hospice as an inpatient or at its day hospice. Services include pain/symptom management, physiotherapy, occupational therapy, complementary therapy, a lymphoedema clinic, spiritual care, counselling and bereavement support. Hospice based in Aylesbury, and service covers Buckinghamshire county, some parts of Oxfordshire (around Thame) and south Bedfordshire (around Leighton Buzzard)

Who

Adults living with life-limiting conditions, including cancer, their carers and loved ones. Referral to the service is made by a health or social care professional
