Charity supporting people living with long-term health conditions, carers and those who are isolated and/or housebound. It connects people to the local community by signposting and provides face to face sessions, an independent support service, group workshops or telephone coaching to help people manage their health. It also runs supervised gardening activities as an opportunity to make friends and learn practical skills, no experience needed. For more information please visit the website.
