Hospice at Home

What

Hospice at Home service providing specialist palliative care to those whose preferred place of care is at home. Support includes attending to people's physical needs, personal care requirements, symptom management, medication administration, general nursing needs, allied palliative care support (including Physiotherapist, Occupational Therapists, Social Worker and Complementary Therapist) and family support.
