Mary Ann Evans Hospice

What

Adult community hospice for people with life-limiting conditions, their loved ones and carers. It offers 7 day palliative and end of life care at home, and bespoke programmes of support at the hospice base in Nuneaton. Services include Hospice at Home which offers personal care at end of life, community support and day respite, 24 hour Rapid Response, wellbeing programmes, Motor Neurone Disease support and complementary therapy. Who

Adults living with life-limiting conditions, who are registered with a Warwickshire GP, their family and carers. Referrals by email to geh.maryannclinical@nhs.net, by any healthcare professional, self or family referral accepted
