Specialist palliative care team supporting people of any age living with cancer, as well as those with other long term conditions. The team aim to help patients and their loved ones live as well as possible by providing pain and symptom control, as well as practical and psychological support. Support and care is given in people's own homes or care homes. You must be referred by a health care professional to the service. Contact for more information.
