Social Prescribing team that works to improve health and wellbeing by linking people to community groups, organisations and services across the health, care and third sector. Link workers take a holistic approach, allowing people to make their own informed decisions about how to achieve their goals and how much support they would like. This may be support with gathering information, setting goals or signposting to arts and crafts groups, peer and friendship groups, gardening and nature groups, and activities that help people to get active. Signposting to practical help and advice services is also possible, from issues such as accessing food, to debt support and benefits advice.
