Social Prescribing in Lincolnshire

What

Drop-in service helping people improve their health and wellbeing in the community. The service works with individuals to discuss their personal needs and then will refer to a link worker. People can then be connected to community groups, activities and services for practical, social and emotional support. See website for more information.
