Social prescribing service supporting you to improve your health and wellbeing. It aims to connect you with wellbeing support in your local area. Social prescribers can discuss your personal needs with you and signpost you to community groups, activities and services to help increase your independence, confidence and wellbeing. East Sussex - the service covers Hastings, Rother, Eastbourne and parts of Wealden (Hailsham and Polegate) and Lewes (Seaford)

Who

Adults aged 18 and over who are experiencing issues affecting their physical or mental health and wellbeing
