NHS service accepting referrals for people living with progressive life-limiting conditions. The service, open daily, offers pain and symptom management, end of life care, rapid discharge from hospital, advanced care planning, and complex psychological or emotional issues (patient/carer). Wigan. Team based at Royal Albert Edward Infirmary, although people are seen at each of the Trust’s sites

Any professional can refer, if patient consent has been obtained. Self referrals also accepted
