Wirral Social Prescribing Scheme

What

Health and community project delivered by Citizens Advice Wirral and Age UK Wirral, to link individuals to a social prescriber. A social prescriber will work with individuals to co-design a personal action plan to improve their health and wellbeing. Activities can help with emotional wellbeing, mental and general wellbeing and levels of depression and anxiety. See website and/or contact for more information.
